How did this patient hear about Coral Reef: Previous patient

Details of experience:

Ms. M was admitted on April 08, 2025, from Homestead Baptist Hospital for recovery due to Chronic Obstructive Pulmonary Disease (COPD). When Ms. M arrived at our community, she was very happy to be around familiar faces. This came after much insistence to her son that if she needed to go to rehab, it had to be Coral Reef. Ms. M is a previous patient, and she felt we took great care of her throughout her last stay. At admission, she shared this is why she chose us, “they come check on you even when you’re sleeping to make sure everything is okay”, and “they try to accommodate your food menu even though it’s not a restaurant.”

Upon her arrival, she was welcomed by familiar faces in nursing, activities, therapy, and concierge service. During admission, she was greeted by her nurse, nursing assistant, and concierge. Within her first two days with us, she was evaluated by our therapy, activities, social services, and activity departments.

After assessing Ms. M, our therapy department determined that she needed to work on her bed mobility transfers and was only walking 20 feet with assistance. After being at our community for over two months, Ms. M is doing independent bed transfers and walking on her own 175 feet. Ms. M also lives at home with her family in a two-story home, where she has been working on her stairs and has advanced to 12 steps without assistance.

Ms. M staying with us has allowed her to participate in many activities offered by our center. Ms. M loves to join us in our recreational department; her favorite activities are bingo and movies. Ms. M will be assisted by our social services team to set up a Home Health agency that will allow her to continue working on her ADLs with an outpatient therapy team.

Ms. M has made a great leap in her progress and continues to demonstrate the desire for future intervention while engaging well with others around her. She always brings a fun energy to the room. We wish her the best in her next journey and hope she continues to be so passionate about her next goal.
